What is Arthritis ?

In Greek, artho means joint and itis means inflammation. In general it is a chronic disease that affects the health of bone joints in the body. It is a very common disease in the United States where one out of every three Americans are affected with this disease. It is seen mostly in aged people who are about 50 and above. It is seen that this disease affects twice as many as women as men. It can be diagnosed through blood tests and x-rays. The disease usually remains for a longer period and sometimes through out the life. It can be sensitive to weather changes.

Types
The various types of arthritis can be identified based on:
• Pace of onset.
• Amount of joints affected.
• Age.
• Sex of the patient.

Based on the above categories different types of arthritis are included in the following
• Rheumatoid arthritis.
• Septic arthritis.
• Osteoarthritis.
• Ankylosing spondylitis.
• Gout.
• Juvenile arthritis.
• Still’s disease.

Rheumatoid arthritis
It is a condition that involves inflammation. It causes the immune system to attack the joints. Due to the joint destruction and pain the patient will not be in a position to move. It also affects the other tissues in the body including heart, blood vessels, skin, muscles and lungs. It is distinguished from other arthritis in which the inflammation and the swelling of the soft tissues of many joints occur simultaneously. The deformities that are involved in rheumatoid arthritis are swan neck deformity, boutonniere deformity and Z-thumb deformity. Blood tests are done to check for arthritis. The main thing checked in blood test in the rheumatoid factor. Treatments for rheumatoid arthritis include anti-inflammatory agents, analgesics and anti-rheumatic drugs.
 

Septic arthritis
Septic arthritis is characterized by inflammation and increase of bacteria in joints. It is suspected when a person is affected with one joint followed by fever. The bacteria penetrate in to the joint through a cut in the skin or carried by bloodstream through any other infectious focus. It is generally diagnosed by laboratory tests wherein it is tested for the presence of neutrophils and other diagnosis done include aspiration, gram stain, culture of fluid from the joint etc. It is usually treated with analgesics, antibiotics or aspiration.

 

Aloe Osteoarthritis (OA)
It is the most common type of arthritis. It is a Greek term wherein osteo means bone, arthro means joint and itis means inflammation. Overall it is a condition where the cartilage, which acts as a cushion inside the joints, is worn out. Due to this the patients suffer with pain during walking, lifting weights etc. It can be divided into primary OA and secondary OA. Primary OA is due to ageing and secondary OA is due to other diseases or conditions. The symptoms noticed in OA are:
• Pain in the joints.
• Contraction of the tendons.
• Muscle spasm.
• Cracking noise when the affected joint is moved.
Diagnosis is done through x-ray tests. There is no proper treatment for osteoarthritis since it is not possible for cartilage to grow back. However, treatments such as anti-inflammatory drugs, glucocorticoid injections are given.

Ankylosing spondylitis
It is an inflammatory disease that mainly affects the spine and sacroiliac joints. It eventually causes the spine to fuse and become rigid leading to a condition known as the bamboo spine. In this case generally males are affected almost three times more than females. It is associated with mouth ulcers, fatigue, eye pain, Reiter’s disease and ulcerative colitis. Diagnosis is done through x-rays to study the affected spine. Although there is no cure for the disease certain treatments and medications are given to reduce the pain and inflammation. Treatments given are physical exercises, TNF  receptors and anti-inflammatory drugs.

Gout
This type of arthritis is formed due to the accumulation of uric acid crystals in joints. It is also called as gouty arthritis. It commonly affects the big toe. It can also affect other areas such as elbow, knee, fingers, spine and wrist. It is distinguished by four stages: asymtomatic, acute, intercritical and chronic. The symptoms are usually:
• Fever.
• Swelling and pain in the joint.
• Warmness of the joint.
• Redness of toe.
The diagnosis includes the testing the patient for Hyperuricemia. Other tests performed are checking for electrolytes, full blood count, kidney function and erythrocyte sedimentation rate (ESR). The treatments given are first and foremost is to reduce the pain. Anti-inflammatory drugs, intraarticular glucocorticoids and other drugs like indometacin are prescribed.

 

Juvenile arthritis (JRA)
This is the most common type seen in children. The types of Juvenile arthritis are:
• Pauciarticular JRA (which affects few joints).
• Polyarticular JRA (which affects five or more joints).
• Systemic JRA (affecting atleast one joint followed by inflammation of internal organs).
The causes of JRA are thought to be environmental and genetic factors.

Still’s disease
It is named after an English physician Sir George F. Still. It is a type of JRA that was often found in children. The symptoms of the disease include:
• Fever.
• Muscle pain.
• Sore throat.
• Skin rash.
• Swelling of lymph glands.
• Enlargement of liver and spleen.
The common diagnosis done is the blood test. 

 

The above mentioned are the primary types of arthritis. The secondary types are given below:
• Psoriatic arthritis.
• Hepatitis.
• Reactive arthritis.
• Systemic lupus erythematosus.
• Familial Mediterranean fever.
• Hemochromatosis.
• Henoch-Schonlein purpura.
• Wegener’s granulomatosis.

Causes
Arthritis can be due to the following:
• Injuries.
• Repetitive motion.
• Strains.
• Falls.
• Sport activities.
• Overexertion.

Signs and Symptoms
Every joint in a human body is surrounded by a protective capsule that holds a lubricating lotion to help in movements of the joints. A slippery smooth substance known as the cartilage covers most joints to make sure that the fluid motion is even. When a person gets arthritis the cartilage would have got damaged, narrowed and lost due to inflammation or degenerative process making the movements painful. Severe pain is the main symptom of arthritis. The other symptoms include stiffness, swelling of joints, sleeplessness, depression, muscle aches, and fatigue. People with this disease may find the normal activities to be difficult like climbing stairs, sewing, cooking, getting up from the bed in the morning, arising from the toilet seat, writing etc. The pain may be severe in the mornings and may reduce gradually. If a person suffers with fever and arthritis of one joint then it is a symptom of septic arthritis. It is emergency and he/she needs to consult the doctor (orthopaedic surgeon) immediately.

Diagnosis
Diagnosis is done based on the type of arthritis. The most often-performed tests include blood tests and x-rays of the affected joints. The blood tests include the following:
• Kidney function.
• Liver enzymes.
• Full blood count.
• Protein electrophoresis.
• Calcium.
• Phosphate.
• Electrolytes.
• Erythrocyte sedimentation rate.
• C-reactive protein.
• Antinuclear factor.
• Rheumatoid factor.

Treatments
If treatments are taken immediately the disease can be cured up to a certain extent. The joints can be protected by lessening the strains. Weight also is an important factor; by being normal weight can reduce the body pressure on hips and knees. The treatments normally involved are: Drug treatments, surgery, reducing stress on joints, physical therapy, occupational therapy and pain management. There are treatments like Harpagophytum procumbens, which is a herbal treatment.
